Extended Health eClaims Policy
We offer direct billing services to most extended health providers. We will work with you and your extended health/insurance provider for your convenience. However, please note that our office is acting on your behalf as a 3rd party agent, information we obtain is very limited, we do not have access to your specific plan and what entitlements you have, and therefore our office is not responsible for discrepancies of your insurance estimates. You will be responsible for any outstanding balances not covered by your extended health/insurance provider. It is your responsibility to understand your insurance plan and to contact your provider directly with any issues related to your policy.
Cancellation & No-Show Policy
We understand that missing appointments can sometimes happen; however, we must consider how these missed appointments impact the ability of other patients to make appointments and access care due to our practitioners’ full schedules.
At Creekside, it is our main priority to give timely, quality care. Late cancellations and no-shows greatly affect this approach to care. To be fair to all our patients and emphasize the importance of committing to your appointments, the following policy is in place:
FULL FEES ARE CHARGED if 24 hours notice is not received for unexcused missed or late cancelled appointments.
IF APPOINTMENTS ARE PART OF A WORKSAFE CLAIM, 3 unexcused missed appointments will result in discharge from the clinic.

Medical Records Policies
Medical records may be faxed by staff members to other doctors’ offices upon request, on a need-to-know basis, particularly when the matter is urgent.
Jane is our electronic medical records and practice management system. Incoming documents are filed in the chart in a logical and organized manner.
Medical records are viewed by staff members on a need-to-know basis only. Medical records will be kept on file for up to twelve years.
